I haven't seen most insanity effects, probably because I always keep my sanity meter above 75%. I thought the one that pretends the game is a shareware or demo version is just plain wrong.

the wierdest one i have experienced is when i was karim, i went through a door, and when i got to the other size i was walking on the roof. another one is when you are walking around and you suddenly begin to sink as though you are in sinking sand.
